         Huachiew Traditional Chinese Medicine Clinic (will be referred in this notice as Huachiew TCM Clinic or our clinic) has realized in the importance of your personal data and others which you are related, you can rely on our integrity and responsibility for the accumulation or the disclosure of your data regarding to 2012 Personal Data Protection Act and related laws. Consequently, our clinic has launched this notice to declare objectives to accumulate, analyze, use, disclose your personal data and the period of preservation along with how to take action on them and your authority on your own personal data

1.     Definition

- Huachiew TCM Clinic means Huachiew Traditional Chinese Medicine Clinic

- Traditional Chinese Medicine is how to use Chinese medicine for diagnosis, examination, treatment, prevention, support and recovery for patients

- Personal data means the data which is used to identify the individual whether directly or indirectly unless who has already died

- Susceptibly personal data means personal data which is designated in Article 26 of 2012 Personal Data Protection Act such as nation, race, political opinion, personal belief, sexuality, crime history, fitness, disability , heredity, biology or others that directly affects to owners

- Personal data analysis means the process on personal data such as accumulating, recording, duplicating, managing, preserving, modifying, adjusting, changing, using, retrieving, disclosing, forwarding, publishing, transferring, merging, deleting, destroying etc.

- The owner of personal data means people who possess data which our clinic has accumulated, used or disclosed

- The supervisor of personal data means a person or legal entity with authority to decide on the accumulation, usage or disclosure any personal data
- The analyzer of personal data means a person or legal entity who accumulates, uses or discloses personal data only by the order or in the name of the supervisor

2. The accumulation of personal data

Our clinic requires only your necessarily personal data when you use our services. The data provided through our website or mobile application or other services such as online appointment, online business, special assistance or the registration at clinic etc. Besides, our clinic can receive your personal data from the third party such as your family, your intimate people, alliances of our clinic or government agents which you have provided them your personal data as the law stated.

3. The collection of your personal data

The type of personal data which our clinic has accumulated even they provided directly or indirectly by owners, depends on the analysis purpose;

- Personal data such as name-surname, photo, sex, date of birth, passport, id card number or any verifiable numbers

- Contact data such as address, telephone number and e-mail

- Payment data such as billing, credit card or debit card and bank account

- Service such as the doctor appointment, family data or other requirements

- News notification, CRS or seminar participation

- Statistics  such as the number of patient or website visit

- The access of clinic website such as IP address, cookies, online appointment system

- Fitness data such as physical or mental health report, laboratory test, diagnosis

- Medical usage or medical allergy

- Feedback and treatment efficiency

- Our clinic will not require your sensitive data such as religious belief, crime history unless it abides by the law or your permission.

4. The objective of personal data analysis

Our clinic will analyze your data with your permission, contract or any acts for the legitimate interest of the clinic for theses following purposes;

4.1 Medical service purpose

- To facilitate patients both online or offline way

- To make an appointment with the doctor, deliver news or recommend new services

- To coordinate or pass the information to other healthcare facilities which will enable referrals faster

- To identify the patient

- To send an appointment notification or present any assistance

- In purpose of accounting or financing

- In purpose of security or safety while using services

- To comply with the regulations of Huachiew TCM clinic

- To comply with the law, regulation or any requirements from the government.

- Other purposes to support all of above or receive occasionally permission.

4.2 Marketing purpose

- To facilitate and present any privileges to patients

- To survey the satisfaction of customer, marketing analysis, statistical analysis to improve products and services or create new ones.

- To be communication channel for Q&A or responding on customer complains

5. People whose disclosed personal data

Huachiew TCM Clinic may disclose or transfer your personal data to third people who are in the country or other countries where personal data protected properly with international standard and law. Our clinic will comply with the necessary and decent standard or rules and laws for all purposes which stated above to these people;

o  Security and safety services

o  Immigration and customs

o  Government agencies or any agencies which allowed or stated in the law

6. The security and safety of personal data

Huachiew TCM Clinic will keep your personal data as long as it is still in need. After that, it will be deleted and destroyed unless the law requires or it protects the benefit of clinic. In general, the period is not over 10 years unless the law requires or other purposes such as preventing violation or financing record.

Our clinic will take the security standard to personal data which covers Administrative Safeguard, Technical Safeguard, Physical Safeguard and Access Control to prevent unauthorized access and disclosure and accord to the operation. Our clinic will revise and improve this policy as necessary to ensure that this data evaluation reaches national standard and regulations

7. The right on your personal data

You may contact to the clinic, officers who protect your personal data or officers who are in charge in order to exercise your right on these following statements;


o  You can access your personal data that secured by our clinic or copy and transfer your personal data to other people or yourself

o   You can object or suspend accumulating, using or disclosing your personal data

o  You can require the clinic to correct, complete, delete or destroy data. In addition, you can require the clinic to expose the source of data in case of no permission      

o  In case you find that our clinic or officers use or disclose your data and it is not subject to your consent or it is not processed according to legal rights. You can complain to Personal Data Protection Committee

o  you can invalidate your consent on data evaluation with our clinic at any time, unless there is a limitation of your right to invalidate your consent by law or by contract to benefit you 

o  However, to invalidate your consent on personal data evaluation can cause data insufficiency to accomplish data evaluation and our clinic may not be able to facilitate you at our best

This privacy notice takes effect from 31 May 2022 onwards
